Requesting Your Records
As part of everwell's transition to new ownership under NuVista Mental Health in October 2025, the organization responsible for the security and confidentiality of your health records (known as your Health Information Custodian) has changed from Carly Fleming, RP #4123, to NuVista Mental Health. This is a routine part of the ownership transition and does not change how your care is provided. Your records remain fully protected under Ontario's Personal Health Information Protection Act (PHIPA).
If you began services with everwell after August 1, 2026, your consent paperwork already listed NuVista Mental Health as your Health Information Custodian, and this notice doesn't reflect any change for you.
How to Request Your Records
Under Ontario's Personal Health Information Protection Act (PHIPA), you have the right to access your own health records, and to request a correction if you believe something is inaccurate.
To request your records:
Speak with your therapist directly, or
Email privacy@nuvistamentalhealth.ca with your name and the records you're requesting
What to expect:
We will respond to your request as soon as possible.
There may be a modest fee to cover the cost of copying or preparing your records - we'll always let you know the estimated cost before proceeding.
If you believe something in your records is inaccurate, you can request a correction using the same contact methods above.
